Public data · 2025 · updated each year The Prevention Gap Map · Michigan

Find the gap. See where to start.

Colorectal cancer screening at every federally funded health center in Michigan, from HRSA's 2025 data. Find a center, compare it with others, and see what closing the gap would take.

01 · Explore

From the whole state to one clinic.

Search a center or a city, filter, or tap a dot. The map, list, table and export all follow the same filters.

At or above targetWithin 10 points10 to 25 points belowMore than 25 points belowRate not published

Read the map carefully. A dot is a clinic site. Its color is its health center's rate; HRSA does not publish rates for single sites. A lower rate is a reason to look at barriers, not a judgment of care.

02 · Compare

Put up to three centers side by side.

Patients, neighborhoods and resources differ, so a comparison starts a conversation; it does not settle one.

03 · Every center against the target

One center, one dot.

The dashed line is the 72.8% target; the solid line is the Michigan rate. Size the dots by eligible adults to see where the most people are.

35 of 39 centers have a published rate. The four without one are not shown here. The Michigan rate uses all 39 centers' published totals.

04 · What it means

Useful signals. Better questions.

These describe the 2025 numbers. They do not explain why a gap exists.

The size of the gap96,170

eligible adults not up to date. Reaching 72.8% means about 45,100 more of them screened.

Move the goal. See what it takes.

Arithmetic, not a forecast: the number of eligible adults stays at 187,620, and nothing is assumed about cost, time or uptake.

06 · The next step · NCQA COF-E, 2027

A screening test is step one. A positive test needs a colonoscopy.

A screening rate counts people who completed a test. It cannot show how many positive stool tests never became a colonoscopy. From 2027, NCQA's new COF-E measure will count that step for adults 45 to 75, within 180 days.

07 · How this is made

Transparent by design.

Checked against HRSA's file on November 5, 2026. Data year 2025; the next update is expected in mid 2027.

Source and period

HRSA Uniform Data System, Health Center Program awardee data, reporting year 2025 (file H80-2025, Table 6B, colorectal cancer screening, adults 46 to 75), refreshed by HRSA on May 7, 2026. Target: Healthy People 2030, objective C-07, 72.8%. HRSA data downloads · Healthy People 2030

What we calculate

Points below target = 72.8 minus the published rate. More adults to target = 72.8% of eligible adults minus those screened, rounded up, never below zero. Michigan figures add up all 39 centers' published totals (91,450 of 187,620). Urban and rural figures use only centers with a published rate. HRSA notes it cannot verify data others reprocess, so these sums are HSREP's.

Rates HRSA did not publish

Four centers have no published rate for 2025. They appear in grey. No rate, gap or count is estimated or shown for them, in any view or export.

Sites and the map

355 permanent clinic sites active on December 31, 2025 (HRSA Form 5B), placed at the center of their ZIP code, so nearby sites can overlap. Every site shows its health center's rate. Names are shown in title case; one spelling in HRSA's file is corrected.

Not a ranking

A lower rate often reflects patients who move often, lack housing or a phone, speak other languages, or live far from colonoscopy, not weaker care. The list is ordered A to Z by default; the planning view orders by the number of adults, for partners deciding where help would reach the most people.
